Create an Incident Case (OSHA)

Start an Incident Case for OSHA 301 through either the Mobile App or Admin Console in Site App Pro.

Incident Case through the Mobile App 

There are two ways to start an Incident Case:

  • Mobile App 
  • Admin Console 

You are given the option of logging an incident via the Mobile App. This could be to capture some quick details from your Foreman or someone on the scene.

To do this: 

  1. Log into the Mobile App 
  2. Navigate to your Home Page
  3. Select "Report Incident" 

This case will then make its way to the Incidents Tab where it can be edited further. More fields are available from the Admin Console so that you can expand on the case. You can also start a new case from the Incidents tab. 

Note: The form that is linked to the "Report Incident" button is being swapped out with a new form. Your customised incident form should still be present in your account, but it won't be linked to the "Report Incident" button anymore. 

To do this: 

  1. Log into the Admin Console 
  2. Go to the Incidents section
  3. Select the Incidents tab 
  4. Select Add new Incident to start a new case

The Incidents Tab allows you to view and create incidents in one spot. Select " Add New Incident" to open the wizard. You can use the wizard to add details in 4 simple steps:

  1. Incident Details (What, When, Where, How)
  2. People Involved (Who)
  3. Resources
  4. Review

Incident Details

Outline what happened, where it happened, and how severe it was. Use this page to include images of the incident and log any corrective actions needed.

Use the Private Toggle to ensure sensitive information remains hidden.

People Involved

Add details about witnesses, injured persons, or investigators. You can select names from your "Team & Users" page or add new ones.

  • People Involved (e.g., injured or ill)
  • Witnesses (optional)
  • Investigators (optional)

When logging the "People Involved", you will be prompted to include the Case Classification. If you enter "Days Away (H)", you will be asked to enter when their date away started and ended. 

This will all pull through to your OSHA 300A Summary automatically. 

Resources

Attach documents or PDFs relevant to the incident. Choose to:

  • Add a new document
  • Add a certificate (with expiry date)
  • Add a link (URL)
  • Link to an existing document
Review

Review all entered information on one page. Use the "Next" or "Previous" buttons to navigate between sections.


Adjusting Incident Status

There are three different Incident Status options:

  • Draft - When incidents are reported in the field using the mobile app, they will automatically be assigned this status. Newly created incidents will also default to Draft
  • Open - Admins can manually change the status of the incident to Open when they are investigating the incident.
  • Complete - Admins can change the status of the incident to Complete when they have completed the investigation and the report.

To change the status:

  1. Click on the Incident to open it 
  2. Select the coloured Status button 
  3. Update the Status
