Need to know why someone has answered "Yes" or "No" to a critical question on your form? You can use the Yes/No + Reason or No/Yes + Reason answer types to make sure you get the full story.
These answer types force a user to provide a written explanation for their answer.

How to Set It Up
You can add this Answer type to an existing form or a new custom form. To do this:
Log in to the Admin Console (Computer Log in).
Go to Forms.
Either click the orange + Add button to create a new form, or click an existing form in the list to alter it.
Click the + Add Question button to add a new question.
Add your Question Text.
Select either the Yes/No + Reason or No/Yes + Reason answer type from the dropdown menu.
Click SAVE.

Examples
Yes/No + Reason
This type forces a user to add a reason if they answer NO.
Question: "Did you induct the new people onto the Site?"
If answered No, they must explain why.
Question: "Have job plans been reviewed?"
If answered No, they must explain why.
No/Yes + Reason
This type forces a user to add a reason if they answer YES.
Question: "Were there any new Hazards today?"
If answered Yes, they must add the details of the hazards.
Question: "Are there any Health & Safety Issues we need to be aware of?"
If answered Yes, they must write what the issues are.

How This Looks in Your Reports
This answer type automatically acts as a Blue Note / Action Alert. When a form is completed using this question, the reason provided will show up in several places:
Activity Stream: The form will appear in the dashboard's Activity Stream, and the reason will be listed as a Blue Note. You can hover over it to read the details.
Completed Form: The reason will be clearly displayed as a Blue Note within the completed form report.
