You can add resource questions (videos, images, PDFs, or links) and signature questions to your Visitor Register forms to improve site safety. Visitors can view safety-related resources and provide a signature to confirm they understand and acknowledge site safety procedures, evacuation requirements, or other critical safety information.
Adding a question to the Visitor Register Form
The Form Builder supports two new question types for Visitor Register forms. You can add multiple questions of any type. While multiple signature questions aren’t common, you might include several resource questions to ensure visitors view all relevant safety information.

To do this:
- Log in to the Admin Console
- Go to Forms
- Go to Visitor Register Induction
- Add question - E.g.:
- Please view the site map and note the first aid & evacuation location
- Please view the Site Health and Safety Procedures
- Select the Answer Type - Selected Resources
- Save & Done!
Please note: if you add a URL resource to a form, the Form Builder checks whether it will be accessible from the Public Register. If there’s a potential issue, a warning will appear. As a workaround, you can attach the video directly to a Resource record and use that instead.
Answering Questions using Visitor Register ( Mobile App or Kiosk Mode)
When a Visitor or Worker opens a Visitor Register form (via QR code or Kiosk tablet), any selected Resource or Signature questions will now be displayed.
To do this:
- The visitor simply taps the resource name to open the image, video, URL, or PDF.
- The Visitor Register will try to display this in a pop-up so the user stays on the form.

In Kiosk mode, this will pop up within the same tab.
In mobile mode, a new tab may open if security restrictions prevent the resource from displaying in a pop-up.
Once a Visitor completes the Form, it is like any other completed form and can be viewed/printed and emailed as usual. for example:
