The Form Assistant allows you to easily build your Health & Safety forms. It includes:
Chat for bulk form changes or building the start of your template
Such as asking for the start of a form
Asking it to make changes across your form (such as Question Type changes)
And, the Upload button allows you to upload a PDF, PNG or JPG file to be converted into a Form.
For example, if you have a PDF copy of a Risk Assessment form, upload it to the Form Assistant. After loading, it will create a form based on the questions found in the PDF.
To use the Form Assistant:
Log into your Admin Console
Navigate to the Forms page
Click the Add button, or click the pencil icon to open an existing form
The Form Assistant exists on the right hand side of your page by default.
To Upload a File into the Assistant:
Click the Upload button
Select a file from your Computer
The File will appear in the chat box
Give a brief description of what you want your form to accomplish
Click ASK
The Form Assistant will add questions to your Form, indicated by Purple Questions
NOTE: Depending on your form size, it can take the Form Builder up to a few minutes to create your form.
Chatting with the Form Assistant:
Once you have a form, you can make adjustments. Simply:
Click on the Chat Box
Ask your Request (E.g. Make all questions in this form Required)
Click Ask or hit your Enter/Return key
Creating a Form From Scratch:
To use the Form Assistant to create a form from scratch:
Change your Context:
The Form Assistant uses pre-loaded context to figure out what regional health and safety legislation to reference in its output.
To customise this:
Click the Toggle/Settings button
Adjust your Location, or Legislation in the fields
Save your changes
Ask the right question:
Like all AI tools, you can optimise the questions you ask to get a better result. When asking for a specific form, provide as much context in the Chat Box as possible.
For example, instead of asking: 'Create a Vehicle Pre-start'
Instead, consider:
What types of Vehicles will this be used for?
Who in your team will be using this form? (contractors? site supervisors?)
And so on,
Use your context to create a form that suits your needs.
A better prompt would be:
"Create a comprehensive vehicle pre-start inspection checklist for commercial trucks and utes, covering engine/fluids, tires, lights, brakes, safety equipment, and body condition, with pass/fail checkboxes and a defects section. Allow the user to add comments if there are issues.
Limitations:
This Form Assistant is not a replacement for good Health & Safety practices. It is merely a tool you can use to make building these templates easier.
Like all AI tools, it is prone to hallucinations. Always double-check the Form Assistant's output and make adjustments.
