Raising a corrective action

For: Admins, Managers, and All Mobile App Users

A Corrective Action is a task used to record and assign follow-up work when an issue is identified. It can be raised directly against a form at the point of completing it, or created independently from the Tasks page in both the Admin Console and the mobile app.

Once raised, a Corrective Action appears in the Task List where it can be tracked through to completion. For a full overview of how tasks work, see An Introduction to Tasks.


From the Mobile App

Linked to a Form

Corrective Actions can be raised against a specific question or against the entire form while completing it.

To raise a Corrective Action against a specific question:

  1. Log in to the Mobile App

  2. Open the relevant form

  3. Tap the three dots icon (...) next to the question you want to flag

  4. Select Add a Corrective Action

  5. Add an assignee and any additional details

  6. Tap Save

To raise a Corrective Action against the entire form:

  1. Log in to the Mobile App

  2. Open the relevant form

  3. Tap the Note icon in the bottom-right corner

  4. Enter your note text in the note field

  5. Tap Add a Corrective Action

  6. Add an assignee and any additional details

  7. Tap Save

Note: If you have already added a note and want to convert it to a Corrective Action, follow the same steps and select Add a Corrective Action. Your existing note text will carry over into the task name field automatically.

Independent from a Form

  1. Log in to the Mobile App

  2. Tap Tasks from the menu

  3. Tap the plus icon

  4. Select the task type β€” Corrective Action or Training

  5. Enter a Task Name

  6. Set the Priority

  7. Enter a Description

  8. Tap Next

  9. Set the Due Date and Time

  10. Select an Assignee

  11. Add any Supporting Files such as a photo or existing resource

  12. Attach any Forms or Resources that need to be completed as part of the task

  13. Tap Create Task


From the Admin Console

Linked to a Completed Form

  1. Log in to the Admin Console

  2. Go to Reports and open the relevant completed form

  3. Click the Clipboard icon in the top-right corner of the form

  4. Select the task type β€” Corrective Action or Training

  5. Enter the task details including assignee, due date, and priority

  6. Click Create Task

Independent from a Form

  1. Log in to the Admin Console

  2. Click Tasks in the left-hand navigation

  3. Click Create New Task

  4. Set the Task Type to Corrective Action

  5. Complete the remaining task details across the three sections β€” Task Details, Schedule & Notify, and Files & Forms

  6. Click Create Task

Tracking and Closing Out

All Corrective Actions are visible in the Task List under Tasks in the Admin Console. Use the Type filter to view Corrective Actions specifically, and update the status as work progresses.

If a form has been attached to the task, the assignee will need to complete it from the Mobile App before the task can be marked as complete. See How to Manage and Complete Tasks on the Mobile App for details.
