Using blue notes

A Blue Note is a way of flagging an issue or exception directly on a form while completing it. It creates a visible record against the form, alerting Admins and Managers that something needs attention.

Blue Notes can be added manually, or generated automatically depending on how your forms are configured.


Adding a Blue Note Manually

There are two ways to add a Blue Note manually β€” against the entire form, or against a specific question.

To add a note to the entire form:

  1. Open the form on the Mobile App

  2. Tap the Note icon in the bottom-left corner

  3. Enter your note text and save

Note: You must complete at least one question before you can add a Blue Note.

To add a note to a specific question:

  1. Open the form on the Mobile App

  2. Answer the question

  3. Tap the three dots icon (...) next to the relevant question

  4. Select Add Note

  5. Enter your note text and save

The note will be linked directly to that question in all reports.


Automatic Blue Notes

Blue Notes can also be generated automatically based on how your forms are set up:

  • Yes/No + Reason or No/Yes + Reason answers β€” when a user answers the flagged option, they are required to enter a reason, which is saved as a Blue Note. See Using Yes/No + Reason or No/Yes + Reason Answers for more detail.

  • Equipment Category limits β€” if a numeric reading falls outside the acceptable range set for a piece of equipment, a Blue Note is automatically added to the form. See Using Limits & Warnings for more detail.


Escalating a Blue Note to a Corrective Action

If an issue needs to be assigned and tracked, you can escalate a Blue Note into a Corrective Action task directly from the form. See Raising a Corrective Action (needs an article) for full details.
